Blue Sky Community Services is seeking to employ professional and values-aligned people to join our supportive and friendly team, to assist us in delivering the My Aged Care Aged Care Assessment Service in the NSW Mid North Coast, New England and North Coast regions. We are seeking Assessors in Taree, Port Macquarie, Coffs Harbour, Tamworth, Inverell, Grafton, Lismore and surrounding areas. These roles are new roles, being offered both part and full-time (30-38 hours per week).

Are passionate about the provision of excellent customer service, and ensuring people are connected to the support and information they need. About the Role The Aged Care Assessor:
Provides person-centred, comprehensive, timely and high-quality assessments that empower participants to access Commonwealth funded aged care services to achieve outcomes across a broad range of key areas. Engages in wellness and reablement focused supports to ensure that participants are able to live as independently as possible in their homes. Provides short-term Linking Services to people who may need additional supports to source and engage supports related to their care needs. Builds the capacity of the community to understand the Aged Care system and associated services. Minimum Requirements Applicants will be required to provide evidence of:
Tertiary qualifications in a health-related discipline directly related to health, aged care or related specialist area. E.g. Registered nurses, medical officers, occupational therapists, social workers. Current registration with Australian Health Practitioners Regulation Agency (AHPRA) or part of or eligible to be part of a relevant professional association. Evidence of current vaccination against COVID-19 and influenza. Please note: A willingness to travel within the region is essential. Applicants will need to have their own fully insured and registered vehicle available for work purposes, with Roadside Assistance.
