Responsibilities: Conduct face-to-face assessments using the Integrated Assessment Tool.
Develop Individualised Support Plans tailored to client needs, preferences, and circumstances.
Liaise with referral sources and community organisations to provide clients with appropriate support.
Complete assessment-related documentation in a timely manner.
Work independently while meeting key performance indicators.
Participate in training under the My Aged Care Workforce Learning Strategy.
Requirements for Non-Clinical Assessors: Diploma or degree in social care, health services, psychology, counselling, or related fields.
Experience in case management, community services, or social work.
Aged care assessment experience, including home or risk assessments for vulnerable populations.
Requirements for Clinical Assessors: Tertiary qualifications in a health-related field (e.g., Nursing, Medical, Allied Health, Social Work).
AHPRA registration or eligibility for professional association membership.
Experience in aged care settings, such as geriatric evaluation, rehabilitation, palliative care, or community nursing.
General Requirements for All Candidates: Must reside in Sydney to conduct face-to-face assessments Full driver's licence and access to a reliable vehicle with comprehensive car insurance (or willingness to obtain).
Up-to-date COVID-19 vaccinations (including booster) .
High-level computer literacy and problem-solving skills.
Awareness of cultural sensitivities and ability to work with diverse communities.
Current National Criminal History Check.
Salary & Benefits Competitive base salary: $80,000 - $116,000 , depending on qualifications and experience.
