This role will see you deliver aged care assessment services to older Australians to enable them to remain living independently at home or access government funded aged care services.

KEY RESPONSIBILITIES:
Conduct home support and comprehensive aged care needs assessments for individuals aged 65 and older.
Develop and review support plans to assist clients in achieving their goals and living independently.
Collaborate with a multidisciplinary team to evaluate clients' physical, psychological, and social needs and determine their eligibility for government-funded aged care services.
Document identified needs and service recommendations for the older adult using tools, templates and the My Aged Care Portal.
Conduct culturally sensitive assessments for groups such as Aboriginal and Torres Strait Islander individuals, those from CALD backgrounds, the LGBTIQ+ community, or the homeless.
Promote healthy ageing and well-being using a 'wellness and reablement approach'.
Be a vital part of a team dedicated to enhancing the quality of life and independence of older adults.
ABOUT YOU
A tertiary qualification in Nursing, Social Work, Occupational Therapy, or a related health field with registration with relevant professional bodies (e.g., AHPRA).
A minimum of one year demonstrated experience in delivering services within aged care settings and/or to older persons.
Experience conducting holistic style assessments using a client centred approach.
Excellent interpersonal and communication skills, both written and verbal.
A passion for promoting independence and well-being among older adults.
Demonstrated ability to use technology such as laptops and/or tablets to facilitate client assessments.
Ability to work within a region where you will be required to travel distances between clients.
Willingness to complete training requirements specified under the My Aged Care Workforce Learning Strategy 2023.
