About the role We have an exciting part-time temporary opportunity for an Age Well Planner/ Care finder to join the team.
Split role between both Age Well and Care Finder, be a part of the development of the New Merri-bek Navigation Service as well as be part of the Care Finder Program which operates across council boundaries with Darebin and Moonee Valley.
The Age Well Planner will be responsible for supporting Merri-bek residents to access the services they need, by increasing their knowledge of My Aged Care, developing an Age Well Guide, providing regular "Ask us Anything" sessions in the community, as well as one-on-one support via phone or in person at local events and festivals.
Regular evaluation and reporting of progress will also be required.
In the Care Finder role, work with older people referred via the Care Finder program across council boundaries within Darebin and Moonee Valley Council areas.
Skills and experience required to perform the role Demonstrated ability to provide support to navigate the service system for older people experiencing homelessness, mental health issues, drug and alcohol issues, elder abuse, neglect, financial disadvantage, and cognitive decline.
The ability to facilitate integrated service coordination and identify new opportunities that will assist individuals to access relevant and appropriate services.
Competence in the use of Microsoft suite of applications, including the ability to utilise an electronic client management system.
Ability to recognise older community members who may have a range of complexity indicators that require specific assessment or service responses.
Knowledge of current Victorian and Commonwealth aged care programs, services, and supports for people over 65 years of age.
High level cultural competency with diversity awareness.
