This is a permanent part-time role 16 hours per week Thursday and Friday located at our Northern Beaches Palliative Care Community Service at Mona ResponsibilitiesAdministration Activities and SupportFollow up and maintain current referral informationPlanning and coordinating appointments for clinicians and booking patients in for MDT using eMRCheck-in and check-out and update patient status in eMRAll patient appointments are entered into eMR in an efficient and timely mannerNotify and confirm patients appointments via email, SMS, letter, and phoneSupport to clients/carers prior to telehealth appointmentsEffective Liaison with LHD/external interpreter Services, coordinating and booking appointments as requiredProvision of accurate Patient Information and update of patient medical recordsProvide thorough training to new or relief staff covering RTU to ensure administration processes are consistentProvide office support services such as photocopying and scanning/faxing, as well as using all office computer systems effectivelySupport reception cover and respond to internal and external enquiries as neededEnter and maintain referral details into the case conferencing databasePrepare weekly patient list for case conferencePrepare and send GP discharge letters as per case conferenceAbout youAble to provide high-level administrative and organizational support in a health environmentDemonstrated problem-solving, prioritization, and communication skillsExperience with Microsoft Office and Health Information Systems (desirable)Comprehensive experience in working in a flexible, multi-skilled team environmentHigh-level interpersonal, communication, and customer service skillsAbility to respond appropriately to challenging behaviors of patientsCapable of working autonomously and effectively following directionsMulticultural sensitivity and/or experience working in a multicultural settingUnderstanding Medical Terminology is desirableA current Drivers Licence and safe driving recordReady to make a change?
