Work side by side with a team of 10 AEP's dedicated to patient-centered care and best practice. Benefit from the experience of our team members with over 30 years of accumulated clinical experience, and specialisations in Exercise and Oncology, mental health, Paediatrics, and musculoskeletal rehabilitation.
Be guided with continual support, mentorship, and ongoing professional growth:
Activate Health has an in-depth New Graduate preceptorship program comprising of time shadowing experienced team members, a dedicated experienced mentor for your first year with weekly 1 on 1 face-to-face meetings, quality reporting templates and systems, and full report writing training and guidance. A gradual introduction to higher skilled services, 3-6 and 12 monthly evaluations, with career mentoring and coaching. Participate in fortnightly AEP case conference meetings including in-house AEP presentations, group case studies, and client brainstorming sessions.

Our services include but are not limited to NDIS, Work Cover return to work physical conditioning, Chronic disease management referrals, home care packages, type 2 Diabetes groups referrals, Private clients, DVA programs, and Health promotion programs. Interventions currently provided include; Mobility and Falls prevention programs for seniors and elderly, NDIS programs tailored to individual needs, including individual and small groups, hydrotherapy, preparation for work-readiness, A large group of paediatric services including in practice individual and small group prescription, Activate's ACTIV Play Kindergarten programs, School Programs, and ACTIV Play and ACTIV Teens Holiday Programs. Activate health's exclusive facilitation in a government funded Health promotion program (At risk identified Chronic Diseases preventative program), delivered to workplaces and the community, Quality Occupational workplace rehabilitation and health promotion programs, Special population classes within our practices including seniors, Parkinson's, hydrotherapy, low mobility, type 2 Diabetes, NDIS, paediatric, and young athletes.
