About Us?
The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The Disability Services Access team is responsible for utilising their skills, experience and attributes in community engagement, person-centered practice, community development and stakeholder engagement to ensure Aboriginal and Torres Strait Islander people with disability aged 0-64 years in service areas of south-east Queensland are individually assisted and supported to have their 'access met' into the National Disability Insurance Scheme and Aboriginal Torres Strait Islanders are supported through their individual pre-plan and plan build cycles.
IUIH is committed to breaking down barriers to access and support Aboriginal and Torres Strait Islander people in an endeavour to address the unacceptably low rate of Aboriginal and Torres Strait Islander participant numbers in the NDIS.
About the Role?
As an Access Officer, you'll provide ongoing support to our members to understand and access support systems including but not limited to the National Disability Insurance Scheme (NDIS), Aged Care Services, and Queensland Community Support Services.
You will play an integral role in the pre-plan and plan build cycle to ensure our clients achieve 'access met'.
We have 2 x full-time permanent positions available.
Within this role, you will be required to:
Work within the IUIH and Member footprints and with designated ACCHOS and existing program networks to establish the most effective means of identifying and reaching the target participant groups of Aboriginal and Torres Strait Islander people with disability and their families.Work with designated IUIH and Member service staff e.g.
Clinicians, Allied Health professionals, Psychologists in the validation of potential participants.Undertake as required targeted awareness/information dissemination program centered on selected clinic footprints.Respond as directed to all NDIS related queries re access/eligibility and general NDIS info from internal and external stakeholders.About You?
Current C Class Drivers Licence (QLD).Current Criminal History Check.Current Working with Children Check (QLD Blue Card) – required prior to commencement.Current Disability Workers Screening Check (Yellow Card) – required prior to commencement.Desirable, but not mandatory:
Demonstrated experience working with Aboriginal and Torres Strait Islander people and/or communities.What Can We Offer?
