Your Opportunity Utilise your lived experience to support people living with severe and persistent mental health.
The Psychosocial Support Program (PSP) is a vital program in the community in partnership with the Healthy North Coast PHN supporting people experiencing with mental illness.
Join us in an exciting role where you will facilitate engaging client activities to ensure effective delivery of psychosocial supports and services.
As an Access Enabler, you will play a crucial part in helping clients access and determine their eligibility for the NDIS, making a meaningful impact on their lives and recovery journey.
Your key responsibilities will be to: Build professional relationships with clients and their support networks.
Record client information and notes in MA Connect.
Develop and review client's recovery and goal plans every three months.
Conduct Kessler 10+ and Kessler 5 and RAS-DS assessments as required.
Help clients access the NDIS, complete forms, gather evidence, and submit documentation.
Engage with NDIA regional contacts to support client transitions.
Transition clients not eligible for the NDIS to other services.
Work with Program Manager, Peer Advocates, and team to maintain partnerships with stakeholders.
Attend regional meetings to ensure integrated program delivery.
Participate in quality and improvement initiatives.
Help build staff NDIS knowledge and participate in knowledge-sharing practices.
Provide reporting and information to PSP management.
Requirements for Success Relevant tertiary qualifications in mental health or related field.
Significant experience in mental health recovery-oriented services.
Experience in facilitating multi-disciplinary coordinated planning.
Experience working with individuals with psychosocial disability and supporting NDIS access.
Strong interpersonal, negotiation, conflict resolution, and partnership-building skills.
Excellent written and verbal communication, administrative, and technology skills.
Proficient time management and adherence to recovery-oriented practices.
Understanding of mental health service systems, policies, and legislation.
Personal experience managing recovery from a mental health condition (lived experience) desirable.
Developing knowledge of relevant legislative requirements.
Understanding of Trauma-Informed Care principles.
