About the Opportunity Are you an experienced clinician ready to take on a leadership role in a dynamic healthcare environment?
Join our dynamic healthcare team in the role of Access and Community Brokerage Manager!
We are seeking a highly skilled professional to provide clinical expertise for individual patients and leadership across your specified area of practice within our innovative Care of the Older Person and Community Transition (CO-ACT) Unit.

In this role, you will:
Deliver and oversee expert clinical care to individual patients.
Provide leadership and guidance to the Access and Community Brokerage Multidisciplinary team.
Coordinate and lead projects or programs that enhance patient, client, and service outcomes.
Exercise significant autonomy in clinical decision-making.
Cultivate strong relationships with both internal and external stakeholders of CALHN to ensure a seamless patient journey into and out of the COACT program.
Accept accountability for your team, including the outcomes for patients or client groups, and ensure consistency with policy.
