Job DescriptionAbout the role You will be responsible for supporting the delivery of the Medical Outreach Indigenous Chronic Disease Program (Aunty Jean's Program).
You will be required to build community capacity through improved self-management for Aboriginal people with chronic health problems, appropriate partnership arrangements and culturally appropriate activities and self-management strategies.
Tasks include but are not limited to the following:
