This is an Aboriginal Identified Position - authorised under the S14d of the NSW Anti-Discrimination Act 1977 About the role We are seeking an Aboriginal Care Navigator for the Medicare Mental Health Centre (MMHC) in Wagga Wagga.
This role works collaboratively with Aboriginal consumers to connect them with services and supports in the community, facilitating person-centred and integrated care.
This position reports to the Service Manager and offers the opportunity to: Provide practical support through information sharing and education.
Identify and address barriers to access, facilitating warm referrals to ensure consumers receive the right service at the right time.
Work closely with other members of the MMHC Wagga Wagga team and external service providers to maximise care outcomes for consumers.
We have a permanent full-time opportunity available, with part time also considered.
Full time employees have the option to enjoy a monthly Rostered Day Off.

Skills and experience We are looking for someone who: Identifies as an Aboriginal and/or Torres Strait Islander and is an active member within the Aboriginal communities of Wagga Wagga and surrounding areas.
Has a tertiary qualification in a relevant discipline, and/or extensive experience in Aboriginal health, AOD, mental health, community services, or social sciences.
Can engage with consumers and their families or carers to provide care navigation and referral support.
Understands the supports and services available in the local and regional communities.
