As a Case Manager/Practitioner, you will play a crucial role in providing person-centred early intervention/prevention support to individuals and families, ensuring they maintain stable housing and thrive in their communities. About the Role: As a Practitioner, Sustaining Tenancies, you will be responsible for delivering essential services to our clients, focusing on prevention and post-crisis support. Utilising a strengths-based approach and Trauma Informed Framework, you will assess and address the evolving needs of our clients, empowering them to sustain their tenancies and enhance their independence. Your work will involve close collaboration with clients, community resources, and stakeholders to provide comprehensive support tailored to individual needs. This a Full-Time Permanent position working 76 hours a fortnight based in Toowoomba, with servicing and travel requirements to between – Toowoomba, Warwick, and Stanthorpe. About You: We are looking for candidates with the following experience and attributes: Proven experience in case management and assessment, particularly within a Trauma Informed Framework Familiarity with early intervention/prevention strategies, including support for families, individuals at risk of homelessness, and vulnerable populations. Knowledge of the public and community housing sector, with an understanding of housing, mental health, substance abuse, and domestic violence issues Excellent written and verbal communication skills, with the ability to negotiate and influence effectively. Collaborative mindset with the ability to build strong relationships with diverse stakeholders. Solution-oriented approach, capable of working independently while also contributing effectively to a team. Proficiency in Microsoft Office suite; willingness to learn and adapt to new software and systems. Required Qualifications and Credentials: Degree in Social Work, Human Services or a related field, and/or equivalent practical experience in the public and community housing sector Current National Police Check Current Blue Card (Working with Children) Current Queensland Driver Licence and access to own reliable vehicle Ability and willingness to travel within and around Toowoomba and Southern Downs service region.
