Role Overview:? Are you a passionate social worker, psychologist, or social science professional looking to make a meaningful impact? SUPPORT NEST is seeking an experienced and empathetic NDIS Case Manager to join our dynamic team. In this pivotal role, you will coordinate and oversee individualised support services, ensuring participants receive the highest standard of care. You will engage in comprehensive case management, focusing on personalised care planning, compliance with NDIS standards, effective risk management, and continuous improvement of service delivery
Key Responsibilities: Comprehensive Case Management: Manage and coordinate support services, ensuring alignment with NDIS practice standards and participant care plans while adhering to budgetary constraints.?Intake and Initial Engagement: Oversee the intake process for new participants, including referrals, screening, risk assessments, and resource evaluations. Collaborate with the team to make informed decisions on service suitability.?Personalised Care Planning: Develop and implement individualised care plans and rosters, considering participants' goals, needs, and NDIS budgets.?Participant Assessments and Reviews: Conduct initial and ongoing assessments, monitor progress towards goals, and adjust support strategies as needed.?Stakeholder Engagement: Build strong, collaborative relationships with participants, their families, support workers, and allied health professionals to achieve positive outcomes.?Team Leadership and Support: Provide guidance and support to Supported Independent Living (SIL) Managers and support workers on complex issues, risk management, and incident resolution. Conduct staff performance reviews and facilitate team meetings and training sessions.?Compliance and Documentation: Regularly review participant documentation, including case notes and incident reports, to ensure compliance with organisational policies and NDIS requirements. Prepare SIL reports and manage NDIS compliance activities.Crisis and Risk Management: Actively manage crisis situations, participant risks, and barriers to effective service delivery, ensuring safety and quality of care.?Continuous Improvement: Develop and implement processes to enhance the quality and effectiveness of service delivery. Stay informed on updates to NDIS policies and integrate changes into service provision.?Communication and Collaboration: Facilitate effective communication strategies within the care team, including interdisciplinary meetings and support worker consultations.
