Why this role matters The Housing Pathways Program (HPP) sits within the Western Metro Homelessness Service and provides housing services and support to connect with the housing system for those in custodial care in the metropolitan Melbourne and Barwon region of Victoria.
About the role We are seeking a values-driven and proactive Initial Assessment & Planning Worker to ensure high quality provision of housing, information and referral services to persons at risk of homelessness, within and exiting custodial settings in Metropolitan Remand Centre. Reporting to the Team Leader, this is a Max-Term (until September 2025), full-time position based in Sunshine, Victoria. Salary and conditions are in accordance with the SCHADS Award Level 5.
You will successfully Assess the immediate and long-term housing needs of service users who are at risk of homelessness exiting designated facilities. Explore and connect service users to appropriate housing options. Provide direct housing referrals to housing providers and refer clients to access points within local area service networks (LASNS), both pre and post release. Assist clients to access support services and establish community supports. You will have A Tertiary qualification in Social Work or related community service field will be (not essential but highly desired). A current and valid Victorian Employee Working with Children Check. A current Victorian Driver licence. The ability to lift 5 kg and carry items short distances when you may be required to take donated items to clients. An understanding of homelessness models, approaches, theories and practices and how to apply them when dealing with clients. The ability to understand and respond to the needs of individuals experiencing homelessness or at risk of homelessness. Knowledge of the Justice system and experience working within correctional settings is considered highly desirable.
