Your new role As Youth Outreach Case Manager, you'll be responsible for delivering strong outreach and holistic case management support for at-risk and vulnerable young people who are homeless or at risk of homelessness across the Casey, Dandenong, Frankston, Mornington Peninsula local government areas. No two days will be alike, but generally, you can expect to:
Work within an integrated services setting to contribute towards enhanced outcomes for young people within their own communities Focus on access and navigation to services and support initiatives, early intervention and prevention strategies and links to broader health, welfare, education, employment and alcohol and other drugs (AOD) service systems. Be central to the day-to-day delivery of Southern Homeless Youth Assistance Program within the framework, ensuring all young people receive equitable opportunities to reach their potential. About you You have experience working with young people at risk or experiencing homelessness. You're experienced in working with young people within a case management framework, utilising a person centred and strengths-based approach. Well organised but able to flex and manage competing priorities and deadlines. Excellent written and oral communication, as well as highly developed interpersonal, coaching and consultative skills. You're a team player, able to work in a collaborative way. Supreme tact, sensitivity, and diplomacy; ability to think on feet and act proactively with discretion. Demonstrable skills and knowledge to work with Aboriginal and Torres Strait Islander people in a culturally safe way. Qualifications Relevant qualifications in Youth Work, Social Work, Community Services and extensive experience in the field; Further study or extensive experience in family work approaches will be an advantage. Current Working with Children card. Level 2 First Aid Certificate. Experience in using SHIP/SRS would be highly regarded.
