Vietnamese Senior Case Manager or Registered NurseApollo Care Services is a provider of care services for both the Aged and Disabled.

We are looking for an enthusiastic Vietnamese speaking Senior Case Manager or Registered Nurse to further develop and grow our business whilst also empowering our broader local community.
Your attention to detail driving the delivery of high customer service excellence with the ability to assist consumer budgets across different packages in either the disability or aged care sector.
Responsibilities Assessing clients' needs and planning, developing and implementing educational, training and support programsInterviewing clients and assessing the nature and extent of difficultiesMonitoring and reporting on the progress of clientsReferring clients to agencies that can provide additional helpAssist in-home assessments, referrals, care plan reviews & goal planning for consumersAssessing community need and resources for health, welfare, housing, employment, training and other facilities and servicesLiasing with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and servicesSupporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutionsPreparing submissions for funding and resources, and reports to government bodies and other agenciesAssisting young people to solve social, emotional and financial problemsExcellent time management with client case loadRespond & communicate effectively with clients re: changing care needsPossess empathy & understanding towards the needs of older peopleWorking within a broader team and liaising with our expert staffTo work within a supportive disability/home care team of dedicated professionalsThorough systems & processes in placeInnovative, progressive community care organisationRewarding permanent position providing quality of life for people within the residential & the communityOngoing training & professional development opportunitiesRequirements for Success Bilingual - Fluent Vietnamese and Professional Working English at the minimumKnowledge of & enthusiasm for Disability and Aged Care Services & a Consumer Directed care environmentDiploma level or above qualification in community, aged, health or another discipline desirableAccess to a reliable, registered motor vehicle with full comprehensive insurance and a valid driver's licenseA clear national police check and working with children check upon role offerIf you are enthusiastic and can demonstrate empathy and understanding towards the needs of the aged and disabled, we would love to hear from you!
