About this opportunity We are seeking a compassionate and dedicated Way Back Support Coordinator to join our team.
In this role, you will provide critical support to individuals in the first three months following a suicide attempt, helping them access non-clinical care and practical resources.
Your efforts will play a vital role in promoting safety, facilitating connections, and ensuring a supportive environment for participants and their families.
Key Responsibilities: Deliver non-clinical care and practical support to individuals and their families.
Collaborate with participants to develop and review personalized well-being plans.
Support engagement in intake and assessment processes, liaising with hospitals and agencies as necessary.
Facilitate connections to health, clinical, and community-based support services.
Maintain accurate electronic records and documentation of participant activities.
Uphold participant rights, confidentiality, and self-determination while ensuring safety planning.
Build and maintain relationships with key referrers and consortium partners.
Participate in monitoring, reporting, and evaluation activities to achieve program objectives.
What do you need: Relevant qualifications or commensurate experience in working in a support role for people with a mental illness, generally a Certificate IV qualification or above Experience in working with people presenting with suicide thoughts or behaviours and who have experienced trauma Current valid Driver's License and the ability to undertake travel for the role Right to Work within Australia Satisfactory Police Records Check [less than 12 months old] and Working with Children Check, prior to commencement – or willing to obtain NDIS (National Disability Insurance Scheme) Worker Orientation Module Certificate is mandatory prior to commencement: https://www.ndiscommission.gov.au/workers/support-for-workers/training-course NDIS Worker Screening Check (NDISWSC) is mandatory prior to commencement About Us Wellways connects people, strengthens families, and transforms communities for individuals of all ages experiencing disabilities or challenges to their social and emotional wellbeing.
