We are currently seeking a compassionate and organized Support Coordinator to join our team in Perth.
Key Responsibilities: Develop and implement individualized support plans for clients.Coordinate and manage services and supports for clients, ensuring they align with their goals and needs.Liaise with clients, families, service providers, and other stakeholders to facilitate effective communication and service delivery.Monitor and review client progress, making adjustments to support plans as necessary.Provide guidance and advocacy for clients to navigate the NDIS and other relevant systems.Qualifications: Relevant qualifications in social work, disability services, or a related field.Experience in a similar role, preferably within the NDIS framework.Strong organizational and time management skills.Excellent communication and interpersonal skills.Ability to work independently and as part of a team.A valid driver's license and access to a reliable vehicle.Preferred Skills and Qualities: Empathy and a genuine passion for supporting individuals with disabilities.Strong problem-solving skills and the ability to think creatively.Attention to detail and a high level of accuracy in documentation.Ability to manage multiple tasks and priorities effectively.Proficiency in using relevant software and technology.Knowledge of local community resources and services.What We Offer: Competitive salary and benefits package.Ongoing professional development and training opportunities.A supportive and inclusive work environment.The opportunity to make a meaningful impact in the lives of individuals with disabilities.How to Apply: If you are passionate about making a difference and meet the above criteria, we would love to hear from you!
