About the opportunity 'No Limits' forms part of IFYS' NDIS service provision, providing support coordination and plan management for service users.
We are seeking to fill the vacancy of Support Coordinator within the Moreton Bay Region.
The NDIS Support Coordinator will implement all supports in a participants plan, ensuring that all funding is fully utilised.
This role is work from home, applicants must be located in the Moreton Bay Region in order to attend ad-hoc client and stakeholder meetings.
What's on offer? Permanent part-time, minimum 45 hours per fortnight, Monday to Friday within business hours, days negotiable Salary base $85,100 per annum pro rata + super Not-for-profit salary packaging options to increase take home pay (package up to $15,900 per FBT year) Work from home, must be based in the Moreton Bay Region IT equipment and work phone provided Free access to the CALM Premium App + Flight Centre personal travel discounts Access to the Employee Assistance Program (EAP) Key responsibilities: Liaise with NDIA in responding to requests for Support Coordination Support individuals and their nominated stakeholders, to understand the supports in their plan and navigate the service system, identifying and accessing mainstream, community, informal and funded support Negotiate services and rates, in accordance with NDIA guidelines and an individual's budget Organise Service Agreements with Service Providers and ensure accuracy Case note activity, complete reports and relevant documentation, monitor individual outcomes and satisfaction with service provision and review expenditure Strengthen and enhance an individual's capacity to coordinate supports, self-direct and manage their plan Key skills and attributes required: Excellent time management and prioritisation skills The ability to communicate with influence and build stakeholder relationships A comprehensive understanding of the NDIS A high level of integrity, a passion for the industry and innovative problem-solving skills Qualifications in Human Services, Social Science or Allied Health desirable Previous experience within disability sector or similar Must hold or be willing to obtain: Paid Blue Card (Working with Children Check) & NDIS Workers Screening Check (or Yellow Card) Child Safety Personal History Check (LCS2) National Crime Check (IFYS will complete a new check through our systems) Tertiary undergraduate degree or higher in Human Services, Social Science or Allied Health required Current Queensland Drivers Licence and willingness to use own vehicle (if required) with comprehensive insurance If you would like further information on this role, please call (07) 5438 3000 and ask to speak to Ameena in the Recruitment team.
