The State Heartprint Lead (QLD) is responsible for ensuring St. Vincent's Care Services innovative model of care (Heartprint) is embedded in all that we do at SVCS. The State Heartprint Lead provides leadership in a kind and considered manner to ensure that homes are supported, enabled, and equipped to deliver true person-centred care with a large focus on connections between residents, staff and residents loved ones.
The State Heartprint Lead aims to enable each home to thrive through the implementation phases of the Heartprint Roll-Out in their embodiment of the model, with support in managing care-related challenges and concerns.
Responsibilities: Be the main point of contact and accountability for the Heartprint Roll-out in QLD.Support facilities to lead site-based Heartprint Champions.Work closely with the Heartprint champions to provide support, advice and guidance on any challenges that are identified in the model's interpretation and delivery.Support with sites-based Orientation on Heartprint education and implementation.Support all facility managers, clinical managers and Living Support Managers on care related outcomes to identify opportunities for improvement.Contribute to quality and compliance KPI's with the facility leaders regarding identified trends and risks.Minimum Requirements: Two plus years of Aged Care Leadership or Management experience is required for this role.Things that work for you: Flexible, Hybrid Working arrangements promoting work/life balance.Competitive salary packaging options up to $15,990, Novated leasing, plus an optional Meals and Entertainment card up to $2,650 per annum.Opportunity to work for the nation's largest Catholic not-for-profit health and aged care provider, with a rapidly growing and progressive vision.Friendly team environment with a great community care spirit.Ability to visibly see your impact on the business and its service.Free annual flu vaccination.Employee Assistance Program for staff and their families.Access to the Fitness Passport (FP).Things that work for us: Experience in an aged care environment.Understanding of Aged Care and resident rights and responsibilities.NDIS Worker Screening Check may be a requirement of this role.Complete Key Personnel Checks (including Australian Securities and Investments Commission and the Australian Financial Security Authority checks).Reconciliation Action Plan St Vincent's Health Australia embraces Aboriginal and Torres Strait Islander people's unique perspectives, knowledge, and cultural.
