We are seeking talented individuals to join our multidisciplinary team in the Community Forensic Mental Health Service (CFMHS). As a Level 3 AHP/Case Manager, you will work collaboratively within a professional and experienced team, providing high-quality mental health care to complex forensic clients. You will be a clinical expert and a vital resource for nursing, medical, and allied health staff, ensuring that we deliver the best possible outcomes for our consumers and stakeholders.The role:In accordance with primary health care principles, Agency policy and direction, legal requirements, service policy and procedures and professional competencies, undertake the delivery of quality patient care services, based on best practice and within a collaborative and multidisciplinary framework.Provide a specialist forensic social work serviceProvide psycho-social assessment and treatment, including the design, development, implementation, and evaluation of treatment programsWork within the multidisciplinary team and participate in case management reviews to ensure continuity of careWhy Join Us?Work in a supportive and passionate multidisciplinary team who are open to innovative ideas and strategies to enhance service delivery and client care.Experience in forensics is not required and there will be opportunities to engage in professional development targeted to our specialised area.Make a meaningful difference in the lives of individuals facing complex mental health challenges while learning invaluable skills that will improve career opportunities.Offices in both Launceston and HobartYou will be a part of a small and highly specialised and supportive team.Flexible workplace arrangements that are family friendly.Details of appointment:Permanent full time, day worker position, working up to 76 hours per fortnight, commencing from as soon as possible.Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.Salary: $106.966 - $118,114 per annum (Pro rata).
