Job DescriptionFixed Term Position: 0.4 EFT (2 x days per week) until 30th June 2025Please note that this role requires working during the 2024-2025 Christmas BreakGreatness starts here! As a Home Care Packages Service Coordinator, you will be the primary point of contact for our clients in our localLGA`s. The role requires exceptional communication and organisational skills with experience working in the aged care sector.
You will be responsible for facilitating client access to services and providing administration support to the Home Care Package team. You will liaise with our home care package clients, internal and external stakeholders, and suppliers to ensure our clients' care needs and goals are met within appropriate time frames.
You will be responsible for:
Providing high level of customer support to home care package clients and the Home Care Packages teamLiaising with home care package clients and all involved in their care and internal and external providers to arrange and manage support services in the client's home and communityProviding timely follow up of referrals and service requests to facilitate optimal care provision for clientsScheduling and rostering of services into Carelink (client management system) and documenting all actions and liaisonsCoordination of appropriate services internally and externally and scheduling of appointmentsMonitoring and maintaining the after hours On-Call phone (weekly rotation paid)Working from a range of DPV Health sites and in the communityDesired Skills and ExperienceTo be considered for this rewarding role you will have: Qualification in community, aged, health or another discipline desirableKnowledge of & enthusiasm for Community Aged Care Services & Consumer Directed careHigh level problem solving, exceptional customer service skills and analytical skillsProfessional telephone mannerA strong customer focused approach to careKnowledge of Carelink + or other client management softwareThe fine print Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), hold a valid WWCC.Level 2 First Aid CertificateOur Talent Experience We kindly request you include a covering letter, addressed to Sandra Reddy, detailing your motivation for joining DPV Health and highlighting the qualities you bring to this position.
