You'll collaborate with clients, their families, and healthcare providers to create personalized care plans that focus on chronic disease management and well-being.

About You A relevant Primary or Allied Health qualification (Registered Nurse preferred)Proficiency in systems such as E Tools, My Aged Care, DEX Data Exchange, GeoOp, Medicare, & ProdaA strong foundation in care coordination and experience with community aged care servicesThorough knowledge of Aged Care and NDIS legislationLeadership skills that inspire teams, paired with outstanding communication and collaboration abilitiesExperience in working with complex health scenarios and managing multi-disciplinary teamsAbility to juggle multiple priorities with easeCurrent Disability Services Positive Notice Card or Working with Children Check (or ability to obtain)Valid Queensland Class "C" Driver's LicenceNational Criminal History Check and up-to-date vaccinations (or ability to obtain)Why Apply?
