The position aims to enhance the health and wellbeing of Aboriginal clients with significant chronic conditions and receiving dialysis by helping to facilitate management of their chronic disease(s) To be considered for this position, you will require: Current registration with AHPRA as a registered nurse or registered Aboriginal Health Practitioner with at least 2 years' post-graduate work experience. Excellent understanding of primary health care and sound clinical knowledge of chronic disease management Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control and knowledge of current issues affecting the lives of Aboriginal people.
