About the role
As a member of the Calvary Community Care team, you will deliver quality services to meet the needs of our clients and contribute to a positive experience whilst at Calvary Community Care. Assist in planning, assessing and evaluating our clients' needs with the help of our multidisciplinary team, to promote positive health outcomes. The registered nurse (RN) position stands within the Transitional Aged Care Service multidisciplinary team. The primary purpose is to provide Holistic Assessment, Clinical Nursing Care and Case Management to clients being supported by the TACS program within the St George Area.
Key Responsibilities Providing clinical nursing care and case management to clients post hospital discharge.Working with a large multidisciplinary team coordinating care with physiotherapists, social workers, Occupational Therapists, dieticians, and Speech Pathologists.Treating and wound care management.Educating clients on medication management and continence. About You
Qualifications in Nursing with current registration with the Australian Health Practitioners Regulation Agency (AHPRA) and current unrestricted Class C driver's licence.
Minimum of 3 years post-graduate experience in an appropriate aged care, rehabilitation or community setting. Demonstrated high level of skills in the comprehensive assessment and management of older people with complex care needs.
Knowledge and experience dealing with aged care, wound care, continence, and dementia issues and management of same in a community setting and/or rehabilitation setting.
Excellent interpersonal, conflict management and time management skills, including experience working with clients from diverse cultural backgrounds.
Ability to work independently and as part of a multidisciplinary team.
Demonstrated understanding and experience in Quality Improvement.
Ability to work within the Mission and Values of Calvary Health Care Kogarah.
