Right at Home Perth Northern Suburbs & Perth West Coast provide a range of care services to assist our aging population living independently at home. This Registered Nurse position is on a Part Time (various hours) working in the Perth Northern and Perth West Coast suburbs.
This role is responsible for:
Clinical delivery and supervision of non-clinical team members in a community setting Intake processing, clinical care planning and reporting Conducting client care assessments with a focus on Consumer Directed Care Coordinate the provision of quality person-centred care according to the client's needs, service/care plans and available funding in accordance with budget Clinical in-home services include care plan assessment, medication, incontinence, wound, chronic disease, complex wound management, palliative care, health and diabetes management General administration for documentation relating to client care and in alignment with AHPRA regulations and legislation Occasional on-call may be required Qualifications ESSENTIAL Qualified Registered Nurse Current AHPRA registered First Aid & CPR Australian National Police check Professional Indemnity Insurance Experienced in assessing and planning care needs within the community Valid Driver's Licence and reliable presentable car A smart mobile phone (iPhone / android) Preferred Skills A passion for high quality client-focused care in a community setting Cert IV in Training and Assessment - highly desirable Exceptional communication skills Able to work autonomously or within a team environment Excellent time management and organisation skills Commitment to ongoing professional development
