JOB SUMMARY
Identifies scanned documents for the electronic health record and analyzes deficiencies by physician or allied health practitioner for dictation and authentication.
JOB DUTIES AND RESPONSIBILITIES
Job Specific :
Ability to operate equipment and computer systems.
Ability to operate the telephone, fax, copier and tube system.
Displays a polite and professional manner, indicating concern and interest with patients, visitors, employees and peers.
Run and print patient discharge list from computer system.
Pick up discharge charts daily from all services areas referencing discharge list.
Prep charts.
Scanning Desktop (EF)
a.
Scan records.
b.
Assign Form identification marking images to discard or rescan when necessary.
Forms Management (EF)
a.
Assign deficiencies and transfer to permanent storage archive.
Medical Record Index (MRI)- Incomplete Record Routine (ICR) Desktop (EF)
a.
Staple any handwritten document to deficiency slip and forward to Incomplete File Area for completion.
Maintains productivity standards of index and analyze and average of 700 images per 8 hours shift.

Monitor pending deficiency list in the Incomplete Record Routine (ICR) routine.

Monitor electronic signature rejection report daily.

Pull records for physician when necessary.
Aid physician in chart completion and desktop navigation.
Monitor and complete deficiency on all electronic and handwritten documentation.

Work Transcription queue and create any needed reports for Supervisor.
Register patient for the Citizens Connect Portal and take calls from patients having problems with portal access.

Cross trained and Certified by the State of Texas to complete birth certificate Acknowledge of Paternity acknowledgements.
Other clerical duties when required:
a.
Pull Advance Directives/Power of Attorney placing in E-Chart for viewing.
b.
Pull paper medical record when requested for patient care.
c.
Use output function of the E-Chart to print, fax records for continued care.
d.
Answers incoming departmental telephone calls in the absence of the department secretary.
Takes messages for release of information and provides messages to release of information staff for processing.
Weekend and holiday duties to include:
a.
Pick up discharge records marking off printed discharge list.
b.
Place discharge charts in accordion folder by patient name.
c.
Prepare charts for scanning.
d.
Release charts by checking "Worklist" from the e-chart desktop several times daily.
e.
Pull paper medical records when requested for patient care.
f.
Aid physicians in chart completion and desktop navigation.
Hospital:

EDUCATIONAL REQUIREMENTS
Required: High School Diploma or GED
Preferred: High School Diploma/GED or medical office management certificate including medical terminology.
EXPERIENCE
Preferred: One (1) year in the operations of Health Information Management
Special Skills and Abilities:
Required: Windows based computer knowledge; basic keyboarding skills.
