Mobile role, covering Kiama and south area
PT role (60 hours/fortnight). Contract ending June 30th 2020

As a Home Support Assessor you will utilise a number of technologies, including the National Screening and Assessment Form, video conferencing, internal social media together with various software products, to support clients and stay connected with your team. You will travel extensively across your region to conduct assessments in various areas and geographic locations. The majority of assessments will be done face to face, with your ability to build rapport second to none. To support client referrals, the ideal candidate will be located Kiama or south. This is a part time role working 60 hours/fortnight on a contract ending June 2020. What will I be doing as a home support assessor? Community based role, visiting older Australian’s in their homes Conducting holistic assessments with a focus on wellness and reablement Capturing vital client information to build on the My Aged Care client story Developing support plans and identifying unique individual goals Connecting people with appropriate services and supports in their community Need to be/have: Knowledge of assessment and goal orientated support planning in aged, community, health or social services Exposure to home support packages Previous experience working in aged care or community services Confident communication with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approach The ability to attend up to 2 weeks of full time training (all costs covered) at our head office in Coolangatta, QLD. Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region Located in the Illawarra area, ideally Kiama or south Comfortable travelling long distances by car What to bring?
