About your opportunity:
In this position you will provide advanced clinical expertise, project management and clinical redesign skills within the Patient Flow Intensive Program, and any other defined patient flow activities. The Clinical Nurse Consultant / Allied Health lead will report to and collaborate with the Nursing Director PFIP and key stakeholder groups to support the implementation of the Patient Flow Intensive Program and associated patient flow and access activities. This role's focus is to identify opportunities across the CHHHS which improve flow and access across the continuum, leveraging clinical redesign principles to affect best practice outcomes.
About your role:
Provide high quality advice in the development and implementation of the Patient Flow Intensive Program initiatives, in collaboration with key stakeholders and the governance committees employing effective change management strategies to facilitate achievement of the project implementation plan and the proposed project benefits.
Provide professional leadership and project management skills within a complex environment, requiring diverse internal and external stakeholder consultation and engagement.
Collaborate with senior management and primary stakeholders across the Hospital and Health Service to implement, monitor and review the effectiveness of the implementation of the Patient Flow Intensive Program; to enhance service delivery and outcomes for our patients across multiple regions.
Prepare high quality documentation of project progress and communication strategies for stakeholder groups inclusive of KPI's and change management principles, with an emphasis on improving and maintaining standards for patient access to care across multiple hospital and health services.
Manage and maintain strategic partnerships with a range of internal and external stakeholders to ensure the delivery of project objectives. Consultation, negotiation and advocacy with other service providers and Managers through networking and collaborative partnerships is a key component of this role.
