As a Nurse Practitioner within CALHN's Integrated Care Geriatrics services, you will bring your high-level clinical expertise to support older people with cognitive, functional, medical, social, and environmental complexities. Collaborating with a multidisciplinary team, you will play a key role in hospital avoidance and supported discharge strategies, ensuring safe and efficient patient transitions to home and community-based services. Your practice will focus on reducing unnecessary hospital admissions and enhancing patient outcomes.
In this dynamic and impactful position, you will use your nursing expertise and leadership to influence clinical care, policy development, and interprofessional collaboration. You will also contribute to clinical education within the organisation, ensuring that contemporary practices are shared across teams. As a Nurse Practitioner, you will be accountable for your nursing practice, professional advice, and delegated care decisions, working within a scope of practice approved by the LHN Nursing and Midwifery Credentialing Committee. Qualifications and Registration: Current or eligible registration as a Nurse Practitioner with the Nursing and Midwifery Board of Australia, including authorisation with AHPRA and a practising certificate with the Nurse Practitioner notation.Skills and Leadership: Strong abilities in autonomous practice, effective leadership, and advanced communication, problem-solving, and change management skills, with a focus on fostering teamwork and innovation.Experience: Extensive experience in managing complex and vulnerable patients, clinical decision-making, triage, and developing out-of-hospital services, as well as mentoring and supervising nursing staff and students.Knowledge: Comprehensive understanding of the Nurse Practitioner role, relevant legislation, professional standards, and contemporary health care issues, with a focus on community-based patient management and safety.If you are a dynamic and dedicated professional passionate about delivering innovative and patient-centred care, we'd love to hear from you!
