Then you are the perfect person to join the Right at Home Southwest Victoria team in Portland! Company Background: Right at Home Southwest Victoria is a local home care provider which started in 2022 and has gained a great reputation already. We provide a wide range of care services all designed to help our clients stay in their homes. Our home care services include companion, personal, nursing, respite, transportation, aged, disability, transitional, 24 hour, and palliative care. Purpose of Position: This role is customer focused and involves consulting with families, preparing in home care assessments, and drafting care plans to best meet client requirements. This position is responsible for your own portfolio of clients and in addition to care co-ordination duties, this position will help develop relationships with hospitals, clinics and care facilities with a view of delivering care services to meet the market. Duties include:Dealing with a range of seniors and frail elderly customers and their families on a daily basis. This role will handle anything from coordinating care, information requests, feedback, general enquiries to complaintsEnsuring all client contact is managed in an empathetic, friendly, interested and patient manner to quickly build rapport, trust and confidence in Right at Home.Providing client and family education on the nature of services that are available and the effect and benefit of the care plan.Preparing in-home care assessments and drafting care plans to best meet client requirementsManaging government funded cases as well as private pay clientsTraining, and managing the performance of the care teamParticipating in community activities or committees as requestedEnsuring highest quality service is consistently deliveredAssisting management with business development opportunities across a range of referral sourcesOperate and maintain care planning and scheduling technologyEnsure all documentation is accurately and comprehensively completed in Right at Home databases and software programs in accordance with organisation policies and procedures and legislative frameworks.QualificationsNational police checkCurrent First Aid and CPRReliable source of transportation requiredComprehensive Car InsuranceIf EEN or RN - Current AHPRA registration & Professional Indemnity InsuranceWorking with Children Check Certificate (Preferred)Willing to gain NDIS Screening#J-18808-Ljbffr
