The role is full-time (Mon-Fri).iChoose is seeking an experienced Support Coordinator to:Think outside the box and work resourcefully with NDIS participants to maximise their NDIS support budget to achieve their goals.Assist participants to gain access to mainstream or community services such as housing, education, transport, health.Build participant capacity so they can exercise greater choice and control and develop skills for increased independence in directing services and supports in the longer term.Attend home and community visits to connect, build relationships and capacity, and ensure that the quality and value of the supports received meets expectations.Ensure all necessary paperwork including case file notes and reporting are completed in a timely manner to comply with NDIS and sector requirements.Advocate for individuals and their families with dignity, communicating their goals and aspirations to their stakeholder team and the National Disability Insurance Agency.Attributes Required:Knowledge of disability services, NDIS, and inclusion principles, as well as a thorough understanding of mainstream and community services or the ability to quickly acquire knowledge.Ability to identify barriers to supports and highly developed skills in working within complex family situations to assist in the achievement of participants' goals.Solid understanding of the impact of disability pertaining to the individual as well as their family and stakeholders.Well-developed communication and interpersonal skills including the ability to manage conflict and resolve/mediate as required.Ability to work with a busy caseload of participants, meet deadlines, and prioritise as required.Ability to positively represent the organisation and to connect with, build, and maintain networks/relationships in the community which maximise the use of available resources and enhance positive outcomes.The ability to work autonomously by managing your own workload as well as the ability to contribute to the Support Coordination team.Strong technology-based skills including client information management system and Microsoft Office suite of products.Ability to work both autonomously and collaboratively with team members and external stakeholders to facilitate seamless service to participants.Qualifications, Skills, and Experience:Qualifications in Community Services or similar (mandatory)Experience in a similar role (mandatory)Blue card and NDIS Worker Screening (mandatory)Experience with Microsoft Suite (mandatory)First Aid and CPR certificates or ability to obtain*Applications without the relevant requirements will not be considered.To Apply:Attach a copy of your current resume/CV and cover letter to register your interest in the position.
