ABOUT US Connections Case Management Pty Ltd is an independent NDIS registered Support Coordination business based in ACT.
We have been operating since 2016 and demand for our services has grown to the point that we now are looking for a like-minded staff member to work with us in the ACT.
We pride ourselves on clients and other stakeholders having had a positive experience working with us, as well as having collaborative relationships with the NGO's and government services who also support our clients.
We work on a person-centered holistic model of care, and we are responsive and creative in our practices to get the best outcome for our participants, to support them in living their best lives.

ABOUT THE ROLE We are looking for someone who has the right attitude and values to join our team to build their skills and confidence to become a superstar support coordinator.
This role will be supporting our team of support coordinators, assisting with the administrative duties, learning the processes, systems, and legislation to take over a caseload of your own when they feel confident.
Remuneration will be in line with skills and experience.
As a support coordinator, your role will include:
Acting as a point of contact between your participant and NDIA/Service providersSupporting senior coordinators in administrative duties.Building rapport, negotiating, developing and/or maintaining mutually beneficial relationships with a diverse range of stakeholders, Local Area Coordinators, or other NDIS Providers to achieve organisational and client objectives.Organising services for each individual client including providing a person-centred assessment and care plan.Planning and/or regularly reviewing client funding and budgets.Maintaining ongoing communication with clients, carers, and families to ensure they are aware of rights and responsibilities to make informed choices.Guiding clients and supporters through the intake process, assisting to follow the processes for NDIS planning.Developing and maintaining positive relations with clients during the length of the relationship between Connections Case Management and their participants from the intake process including up to and including the service agreement process and ongoing management of service provision.Preparing reports and documentation on current and predicted demand.Compliant management of Connections Case Management systems and processes.Maintaining software systems.Managing client, caregiver, and external advisor emails and phone calls.Educating and empowering participants to exercise their choice and control while learning and building skills on how the NDIS works.ABOUT YOU Must be familiar with the ACT and its services - essential.Evidence of a current valid Driver's LicenceOwn car with comprehensive care insurance.Willing to obtain NDIS Worker Screening Check and ACT WVPStrong belief in the right of People with Disability to live their best lifeDriven to get the best outcome for participantsRelevant qualifications in mental health and disability or extensive experience in working in the NDIS space with this group of clientsProfessional approachExceptional organisational and time management skillsExcellent attention to detail and a confident manner.Positive approach to changeBe fully vaccinated against Covid-19.If this role sounds like you and you would like more information in the first instance, please send your resume with your covering letter addressing your suitability for this role to .
