Our goal is to enable rather than do, to promote independence, development and confidence of the individuals. Our success will enable them to have the voice and choice to thrive in their local communities.The role entails:Provide 1:1 & 2:1 support to consumers in their own home and community setting.Meet the individual needs according to an established behavioral support plan.Develop professional relationships with individuals, their families and others in the community.Provide accurate and timely case notes so that others can provide a consistent experience for the individual.Provide support to assist consumers to work towards their individual goals.We provide 24/7 support to our consumers and are seeking casual staff to join our Casual Workforce Team.
