Your new role We are currently seeking an experienced and motivated individual who is keen to join our organisation as HHS Worker on a Part Time basis for 22.5 hours per week.
Reporting to the HHS Coordinator, you will work collaboratively with other support workers and teams to ensure a collaborative team culture is maintained.
In this dynamic and multifaceted role, you will ensure that service delivery practices are consistent with Wintringham's philosophy, values and mission.
Your responsibilities and duties will include: Conducting initial assessments of new residents upon their entry into the housing programDeveloping and documenting Support Plans as appropriateConducting and documenting ongoing client assessmentsIdentifying gaps and supporting needs and work to develop appropriate responses and strategiesWorking with clients by linking and referring them into clinical services and social and recreational activitiesEnsuring there is a pathway for clients to register and access higher intensity support through Home Care Packages (HCP) and/or NDIS by assisting with referrals and case managementCompleting/reviewing client file documentation across program areas to ensure timely and accurate recording of informationAssisting clients to maintain the lifestyles of their choice whilst maximising their health and wellbeingAddressing issues of social isolation for clientsAssisting clients to maintain their tenancies and long-term accommodation and therefore prevent premature entry to residential aged careManaging client feedback and complaintsUtilising relationships within sectors to assist access into other services and systemsWe are looking for candidates with the following: You are a proven leader with a track record of success in the leadership of homelessness services, delivering operational results, building amazing cultures and achieving budget targets.
Your skills will be supported by possessing the following:
Qualification in Social Work, Welfare, Community Services or equivalentProven senior operational management experience within a diverse and complex community services environment.Thorough understanding of Commonwealth and State Government policies, standards and compliance requirements.Demonstrated skills in financial management, including budget development, monitoring, implementing corrective actions to resolve cost escalations and reporting of variances.Advanced skills and experience in program design and identifying, documenting and implementing quality systems within a continuous quality improvement framework.Demonstrated commitment to social justice and social inclusion and the development of a strong Community Sector, including knowledge and understanding of the causes of homelessness and related social policy issues for older people.Operational understanding of the courses and the effects of accelerated ageing, frailty, and vulnerability factors for people who are experiencing or with histories of homelessness.This sounds great!
