Permanent full-time opportunity to supervise and lead a team providing non-clinical home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT).
About you Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Lead a team providing support services to determine the priority needs of senior Queenslanders, including the assessment of aged care requirements and a summary of assessment findings, goals, and recommendations.Supervise, mentor, and support new home support assessors within the Single Aged Care Assessment Team, for all elements of day-to-day business related to assessments.Undertake Home Support assessments for Commonwealth Home Support Program services to meet the ongoing needs of the older person.Identify clients who would benefit from a Reablement Focused Program and action referrals to the appropriate services.Contribute toward quality improvement activities that will support the availability of assessors, management of workloads, and quality of outcomes for clients.Health Equity It is expected that all Metro North Health staff, including the incumbent of this role as a valuable member of the Metro North workforce, contribute to the health equity agenda and meet the intent of supporting the defined six actions that specifically meet the needs of Aboriginal and Torres Strait Islander people within the National Safety and Quality Health Service Standards (NSQHS), by actively supporting the elimination of racial discrimination and institutional racism; supporting increased access to health care; influencing the social, cultural and economic determinants of health; supporting the delivery of sustainable, culturally safe and responsive health services; and recognising the importance of working with Aboriginal and Torres Strait Islander peoples, communities and organisations to design, deliver, monitor, and review the health and support services we provide.
