We are looking for full-time HCP/CHSP Coordinator to join our Aged Care team servicing the Clarence Valley. This position is based in Grafton on a 12 month fixed term full-time contract.

As a HCP/CHSP Coordinator with Livable you will: Help customers to understand and maximise their HCP budget funding.Assist customers to navigate their support plan and set up services.Manage and monitor each customer's HCP budget in accordance with applicable service agreement and support plan.Develop and maintain professional, customer-centred relationships and partnerships with key stakeholders.To be successful, you will need: Appropriate qualifications in Aged Care or Disability.Ability to work with a diverse range of customers and stakeholders, including the aged, people living with a disability, cultural diversity and language other than English, and the ability to adjust ways of working to support best-practice inclusion and accessibility.Excellent customer service skills with a passion for ensuring the best care outcomes for customers and their families.Exceptional communication skills (clear and concise written and verbal communication).Good computer skills and experience using customer CRM systems.Self-motivated with a strong sense of accountability and the ability to take ownership of tasks and the customer experience.Ability to work independently but also as part of a team.You will also be required to complete the NDIS Quality & Safeguards Commission Worker Orientation Module or provide evidence of completion. The successful candidate will also be required to demonstrate entitlement to work in Australia and to disclose any pre-existing injury or disease that may be adversely affected by undertaking the inherent requirements of the position.
