Full Time Home Care Package Advisor Hervey Bay Due to continued growth of services, we are looking for an experienced and motivated Home Care Package Advisor to join our Wide Bay Team. This role is instrumental in providing support and advice to the Home Care Packages clients to assist them to continue living at home. Your responsibilities will include day to day management of a client case load, conducting assessments, care planning and provision of consumer directed services to complex care clients within budget guidelines. You will build professional relationships internally and externally and look for creative care solutions to achieve the best outcomes for clients. This role involves travelling to meet clients in the community as well as working from home and occasional visits to our Bundaberg site office.
JOB PURPOSE The Home Care Package Advisor performs an integral role in assisting in individualised services being delivered to customers. 
DUTIES AND RESPONSIBILITIES  Assist in customer service schedules including service planning, confirmation and short notice placements or cancellations.Liaison with Aged Care Assessment Team (ACAT), My Aged Care (MAC) Portal and other regulatory bodies.Ensure care plans are monitored and reviewed on a regular and ongoing basis and in response to changes in customer needs.Ensure services are delivered are in line with the person centred and consumer directed principles and budgets.Assist customers to access other alternative services where the services are no longer meeting their needs.Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectives.Monitor customers' budgets in accordance with program guidelines.Act on care need recommendations by Health Care Practitioners.Maintaining all required administrative documents, records and customer notes, compliance spreadsheets and Home Care Package documentationIdentify and participate in personal professional development.Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.Communication with key stakeholders, internally and externally.Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.Any other duties as required to meet the ongoing needs of the organisationQUALIFICATIONS, KNOWLEDGE OR EXPERIENCE REQUIRED  Qualifications in Community Services, Human Services, Nursing or equivalent.Previous experience in Aged Care or Community Services.Ability to work flexibly to respond to changing customers' needs.Ability to coordinate services according to relevant program guidelines.Experience of working in a multi-disciplinary team.Ability to develop and maintain links and supportive networks with specialist services.Experience maintaining computerised customer information systems.High level administrative and organisational skills and time management skills.High level of written and verbal communication skills.Ability to be a team player and work as part of a larger team.Demonstrated ability to contribute positively to the team.Current and clear Criminal Record Check / NDIS Worker Screening Check.Current Working with Children Check (where required in accordance with state based legislation)Current Drivers Licence.ATTRIBUTES AND SKILLS High level communication skillsPlanning, organisational and time management skillsResilience in a fast paced environmentComplex Problem solving and ability to prioritiseEmpathy and compassionRelationship Management – internal & external stakeholders Just Better Care recognises the value of equal employment opportunity and is committed to promoting fairness, equity and diversity in its workplace.
