About the Role
As a Care Coordinator you will assess eligible, high-risk clients living with complex healthcare needs.
The primary aim of this position is to provide generic care coordination in ED and the community that facilitates the client's self-management strategies in order to improve health outcomes and prevent avoidable hospital presentations.
As an Occupational Therapist, you will provide assessment, intervention and management to achieve optimal functional recovery, encourage self-management and promote empowerment for clients to improve their health outcomes, wellbeing and quality of life.
Key duties are:
Early identification of ALERT patients in the Emergency DepartmentProvide appropriate generic and discipline specific clinical management, discharge planning & community liaison for all relevant patientsTo engage with and provide outreach care co-ordination services for identified or referred clients including ensuring holistic assessment and collaborative care planning to guide interventionsProvision of clinical interventions such as functional retraining, equipment prescription, home modifications, falls prevention, pressure care, energy conservation and remedial strategies and make referrals for ongoing follow up/long term intervention as required.Classifications for this position will range from classification range Occupational Therapist Grade 2 VF6-VF9 ($90,235.6- $103,724.40 per annum).

The program has a base of services within St Vincent's that currently supports clients that are experiencing or at risk of homelessness.
This service aims to build on integrating and better coordinating existing services within St Vincent's Hospital and the development of new services to meet the need of the target client group.
ALERT key target groups include patients with complex needs who fall under one or more of the key risk factors; substance use, mental health issues, homelessness, disability and complex aged care issues, family violence.
This position is based at St Vincent's Hospital Melbourne within the Emergency Department and also providing support and interventions in the community as part of an interdisciplinary team.
