About the role: As a central point of contact for support workers and participants, the Relationship Manager role is core to what makes Hearth different to other support providers.
Relationship Managers ensure that participants, their family members, support workers, and all other stakeholders have a clear & consistent point of contact.

Join a rapidly growing organisation with a warm and supportive cultureSmall, fun, & supportive team based in Central BendigoFlexible working hours and the option to work remotelyOpportunities for career development and progression to other roles within the organisationTravel compensation, work laptop, and mobile phone providedResponsibilities will include: Creating and continuously developing relationships between participants and their team of Hearth Support WorkersAssessing participant needs, and developing Hearth's support programMaintaining relationships with support coordinators, house supervisors, and other relevant external stakeholdersLead the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goalsReporting on the progress of clients, as it relates to the support provided by Hearth, and preparing submissions for NDIS plan reviews as requiredCoordinating a weekly staff roster for each participant and proactively resolving rostering issues.Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS planCompliance with Hearth policies and procedures to ensure a safe working environment and detailed, timely incident reportingRequirements: Proven experience working in the disability sector, preferably in a supervisory or managerial roleA Professional level of administrative and business communication skills, including case noting and report writingExcellent time management and organisational skillsProven client centred approach with the ability to advocate for Participants and an understanding of complex disability and traumaOwn car and driver's licenseStrong interpersonal skills and high levels of resilienceUnderstanding of the NDISWorking knowledge of Microsoft office suite and employee rostering systemsPlease include a copy of your up-to-date CV in your application, along with a Cover Letter addressing the key selection criteria.
