We achieve this through fostering and cultivating long-term and sustainable relationships.About The RoleAs a central point of contact for support workers and participants, the Relationship Manager role is core to what makes Hearth different to other support providers.Relationship Managers ensure that participants, their family members, support workers, and all other stakeholders have a clear & consistent point of contact.Why work at Hearth?Join a rapidly growing organisation with a warm and supportive culture Small, fun, & supportive team based in Central Bendigo Flexible working hours and the option to work remotely Opportunities for career development and progression to other roles within the organisation Travel compensation, work laptop, and mobile phone providedResponsibilities Will IncludeCreating and continuously developing relationships between participants and their team of Hearth Support Workers Assessing participant needs, and developing Hearth's support program Maintaining relationships with support coordinators, house supervisors, and other relevant external stakeholders Lead the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals Reporting on the progress of clients, as it relates to the support provided by Hearth, and preparing submissions for NDIS plan reviews as required Coordinating a weekly staff roster for each participant and proactively resolving rostering issues.
Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS plan Compliance with Hearth policies and procedures to ensure a safe working environment and detailed, timely incident reportingRequirementsProven experience working in the disability sector, preferably in a supervisory or managerial role A Professional level of administrative and business communication skills, including case noting and report writing Excellent time management and organisational skills Proven client centred approach with the ability to advocate for Participants and an understanding of complex disability and trauma Own car and driver's license Strong interpersonal skills and high levels of resilience Understanding of the NDIS Working knowledge of Microsoft office suite and employee rostering systemsPlease include a copy of your up-to-date CV in your application, along with a Cover Letter addressing the key selection criteria.
