Based at our Clarendon Clinic, which is centrally located and close to multiple public transport routes, this is a great opportunity to increase your skills in community mental health.
This is a community based role in an innovative environment, working with clients who may be homeless or at risk of homelessness who suffer with a serious mental illness and associated high level of disability.
You will join a dynamic Homeless Outreach Psychiatric Service operating under a recovery-oriented model of care, utilising the Strengths Model of Case Management.
Work within the Strengths Model of recovery orientated practiceBuild a therapeutic alliance with the consumer and family/carers and actively engage them in the planning, delivery and evaluation of their careUndertake the assessment, provision of treatment and delivery of targeted interventions to consumers with complex mental health related needs and their family/carers in line with SVMH policies and strategic directionWork a 7 day rotation rosterClassifications for this position will range from RPN Grade 3 NP81 - NP74 ($100,932 - $112,091 per annum)

The Homeless Outreach Psychiatric Service provides ongoing specialist treatment and support to meet the identified needs of clients who suffer with a serious mental illness and are homeless, or at risk of homelessness in the Cities of Yarra and Boroondara.
The multidisciplinary team provides a range of clinical and therapeutic interventions using the recovery-focused Strengths Model of Recovery-Oriented Practice.
