Prince of Wales Hospital is seeking a CNS grade 2 for a temporary part time position funded by the Central and Eastern Sydney Primary Health Network (CESPHN) for 12 months for the implementation of the Diabetes Dashboard.

The Prince of Wales Hospital is a major teaching hospital based in Sydney's eastern suburbs that serves all of New South Wales. The hospital is situated close to the city, Bondi Beach, excellent shopping facilities, and within walking distance to Coogee Beach.

We are affiliated with the University of NSW, one of the premier medical teaching facilities in Australia. This assists us to provide excellence in care in conjunction with our commitment to clinical teaching and medical research.

Currently the NSW Government is investing $720 million to redevelop the Prince of Wales Hospital and progress the vision of Randwick as a world-leading centre for health and wellbeing, research, education and teaching. An Acute Services Building will be built in Stage 1 including a new adult emergency department, extra beds, intensive care unit, and new operating theatres for the campus.

More than just a new hospital building, this major redevelopment will transform how we deliver health services at Randwick. This facility will enable us to continue to attract and retain the best nurses, doctors and support staff and meet the changing health needs of our community. The primary purpose of the Clinical Nurse Specialist Grade 2 (CNS2) Diabetes Dashboard is to assist the Endocrinology Medical Team in the implementation of the Diabetes Dashboard at Prince of Wales Hospital. The CNS2 will provide education and support to inpatients and outpatients with diabetes identified from the Diabetes Dashboard, including participating in data collection. The CNS2 will also attend multidisciplinary case conferences and provide phone assistanceadvice to patients with diabetes who have been linked to the Diabetes Centre identified from the Diabetes Dashboard, including working with other Diabetes Educators.
The CNS2 Diabetes Dashboard will also provide support to patients of Aboriginal and Torres Strait Islander background identified through the Diabetes Dashboard, and recruiting them into a study which examines the feasibility and acceptability of providing diabetes education support to these patients. Activities include providing diabetes education, communicating with GPs and other in and out of hospital staff and linking patients to appropriate community services, and documentation of these activities and outcomes. SESLHD is committed to creating a workplace that reflects the diversity of our community.  Selection CriteriaCurrent registration with the Nursing & Midwifery Board of Australia (AHPRA)Holds Post graduate qualifications in diabetes nursing and at least 3 years experience in acute care nursingDemonstrated ability to manage complex clinical care and service delivery issues and problem solve through the development and use of clinical management plansDemonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situations.Demonstrated skills in the development & facilitation of patient and carer education and staff as required.Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others.Detailed knowledge and understanding of harm minimisation and evidence of leadership in safety and quality innovation.Demonstrated competence in the use of health-related information technology.
