Both of these options remain valid for a period of six (6) months from when the authorised delegate endorses the recruitment decision. Position Profile: This role works with the aged care service at Rockingham General Hospital (RGH) and primary health care providers to identify, assess and support patients at risk of unsuccessful discharge from hospital, supporting and monitoring the transition of care from hospital to primary health care providers. The successful candidate will work collaboratively with geriatricians, general practitioners, nursing colleagues, and allied health staff to coordinate and support effective discharge activities, develop long term care plans, and educate and enable patients to manage their health and social care. Please note, the occupant of this position will be employed as a continuous shift worker.
