Limestone Coast Local Health Network - Country Health Connect, Mount Gambier Salary: RN/RM2C $84,870 - $107,279 p.a.
(Pro-rata) Permanent Part time position working 19 hours per week Overview As a key member of the multidisciplinary Country Health Connect team, you will provide a comprehensive community and inpatient based Diabetes Program in the Limestone Coast region.
You will develop health promoting strategies for people with diabetes, coordinate the Diabetes Program, and liaise with other health professionals within the Limestone Coast Local Health Network (LCLHN) to ensure the highest quality evidence-based care for clients.
This will include undertaking diabetes education and nursing practice and using primary health care strategies for individuals and groups.
Monitoring, promoting and fostering a culture centred on client services and a team approach to work practices, you will facilitate staff development, client teaching and nursing research with a focus towards the prevention of and management of diabetes.
You will be expected to maintain a close working relationship with the Diabetes Nurse Practitioner and other Diabetes Nurse Educators within the region.
You will be registered or eligible for registration as a Nurse and/or Midwife with the Nursing and Midwifery Board of Australia, and be eligible to hold a current practicing certificate.
With demonstrated competence in diabetes nursing in accordance with the appropriate standards of practice, you will have experience in the leadership and direction of student nurses, Enrolled Nurses and less experienced Registered Nurses.
Effective communication, problem solving, conflict resolution and negotiation skills will be essential.
You will bring the ability to engage with Aboriginal communities and consumers in a culturally appropriate manner and be willing to undertake further training in this manner.
