It is aimed at older clients, but age alone is not an eligibility criteria.The position of clinical nurse (Case Manager) will have responsibility for the assessing, care planning, provision of nursing care, coordinating and monitoring of allocated clients within the Transition Care Program. It also involves case management leadership and partnership working with Non-Government Organisations and external service providers who provide care and therapy to Transition Care clients.Permanent Full Time (76 hours p.f.) or Permanent Part Time Positions (hours negotiable) We encourage applications from First Nations nurses and midwives in line with Health Equity, Growing Deadly Families, and Hospital and Health Plan Strategies. You will receive a competitive remuneration package value up to $131,900 p.a., comprising salary between $107,960 - $115,604 p.a. (f/t), or salary rates between $54.45 - $58.30 p.h. (p/t), employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)) (Permanent Full Time, 76 hours p.f., or Permanent Part Time Positions, hours negotiable).
