About the opportunityAt Mercy Health, each moment, no matter how small, truly matters - and as a Clinical Care Co-Ordinator, you'll create cherished moments each day.You will be responsible for coordinating and managing patient care plans, effective communication between healthcare providers and residents while monitoring progress to optimise patient outcomes and quality of care.

Strong leadership experience within the health, disability, or home care sectors.Sound knowledge of the aged and disability sector systems and funding, business acumen including financial literacy.Excellent time management, organisational, and communication skills, with the ability to adapt and problem solve.A compassionate, flexible, and positive approach to effective patient care and management.A safe, sensible approach with a practical understanding of people's physical limitations.The ability to be flexible and adapt to meet clients' individual needs.We would also love you to have (or be in the process of obtaining):
