Due to significant growth, the Home Care Packages team we have an opportunity for a casual / locum **Case Manager **to assist with peak demands of intake and backfill duties when permanent staff take leave. If you wish to move away from shift work, or working full time - this may be the job for you!
As a** Case Manager** within the Home Care Packages division you will work within a client directed framework with a focus of ensuring the wellbeing of older people who are in receipt of a Home Care Package. You will be responsible for developing, reviewing and monitoring care plans with participants that meet their individual needs and identify their individual goals. You will also actively engage and collaborate with a range of internal and external services partners to ensure a holistic service is delivered.
When required for backfill purposes, you will have capacity to flex your hours, whether you wish to work full time, part time, school hours, working a blend of from home and conducting client visits. Commencing in a casual capacity, there may be opportunities to move to permanent employment in the future as vacancies arise.

**About You?**:
Our ideal** Case Manager** will demonstrate the following:
- Understanding of the social, psychological and physical aspects of ageing and disability including the role of carers and a commitment to the principles and practice of client directed services;
- Experience in community care, particularly care/case management;
- Knowledge, skills and experience in the delivery of behaviour management strategies;
- Ability to be flexible, innovative and independent as you respond to the changing needs to care recipients and team members.
You will need the following:
- Relevant Tertiary qualification and/or extensive experience in a human services discipline;
- Current Drivers Licence;
- Current NDIS Worker Screening Check or willingness to obtain;
- Current NDIS Worker Orientation Module certificate or willingness to obtain;
