About the role We are seeking Care Navigators to support the establishment of the Medicare Mental Health Centre (MMHC) in Wagga Wagga.
This position is a permanent, full-time opportunity.
We will consider part time applications.
This position reports to the Service Manager, and offers the opportunity to: Provide person-centred, practical support by connecting consumers with appropriate services and community supports.
Facilitate access to care, ensuring consumers are linked to the right services at the right time.
Work collaboratively with a team of mental health professionals and external service providers to deliver integrated and holistic care.
Address barriers to accessing services by sharing information, providing education, and facilitating warm referrals.
Support consumers throughout their journey, acting as a key point of contact.

Skills and experience We are looking for someone with: Tertiary qualifications in mental health, health, community services, or social sciences or extensive experience in a similar role.
An understanding of the health and wellbeing needs of individuals with mental health and/or substance use issues.
A solid knowledge of community and regional services available to support consumers in Wagga Wagga and the surrounding areas.
Strong communication and interpersonal skills, with the ability to engage with consumers, their families, and carers.
Current Australian driver's license and willingness to travel as part of the role.
