An exciting opportunity has become available for a motivated, self-driven Care Advisor to join our team. This role is responsible for building relationships and implementing a strong customer focused culture.

Key responsibilities include:

- Meet, engage and build relationships with clients to develop, refine and achieve their goals.
- Provide information and support to clients and their families/carers to monitor the use of their funds.
- Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS plan.
- Conduct annual reviews for all clients - revise care plans, goals, service provisions to quality standards and budget expenditure and maximization.
- Ensure service provisions are conducted ethically, fairly and within the statutory legal and contractual requirements at all times.
- Compliance with Health policies and procedures to ensure a safe working environment and detailed, timely incident reporting.
- General office administration duties.

The successful applicant will have:

- Understanding of NDIS and home care packages (HCP).
- Qualifications in Nursing, Allied Health or Social Work.
- Previous case management experience.
- Demonstrated knowledge of the Aged Care funding model.
- Exceptional customer service, negotiation and influencing skills.
- Excellent ability to connect with multiple stakeholders and think commercially.
- Strong interpersonal skills and high levels of resilience.
- Driver's license and access to own vehicle.
- Professional verbal and written communication skills.
